For insurance coverage for diving activities, all dives during university travel or performed as part of your job duties (descent underwater utilizing compressed gas and subsequent ascent/return to the surface) must be reported to the Office of Risk Management using the form below.

All divers should review the diving standards established by The American Academy of Underwater Sciences for diving safety best practices.
Non-LSU employees are not insured for diving activities by the University and should purchase appropriate coverage.
Exceptions to completing this form may be submitted to Risk Management with a copy of your written job description that requires diving as a routine job duty.
